Timing: If your cruise ship docks at 7am and departs at 7pm, you have a solid 10-hour window. Rome is doable but tiring. The closer destinations (Tarquinia, Santa Marinella) are more relaxed and let you return to the port with time to spare.

Food: Don't eat at the port. Walk 10 minutes into Civitavecchia town for far better (and cheaper) pizza, pasta, and seafood. The restaurants along the Lungomare waterfront are solid.
